ADMISSION CRITERIA

New residents to Parents’ Foundation for Transitional Living, Inc. are admitted to the Residential Living Center.

Admission is open to adults, age 18 and older, who:

  • have a primary psychiatric diagnosis

  • are evaluated to have the potential to live in the community and benefit from our program

  • have basic self-care skills

  • are able to abide by safety rules, and

  • provide the required clinical information

Parents’ Foundation for Transitional Living, Inc. is not a setting for individuals who are suicidal, assaultive, have a primary substance abuse diagnosis or a dementia.

Referral Procedure
Referrals may be made by contacting our Clinical Director by phone at 203.776.3236 (or visit our Contact Us page on this website).

The Clinical Director is available by phone to discuss an individual’s potential program participation. For those interested in pursuing admission, the Clinical Director will send (by mail, fax or email) an application for admission, a part of which must be completed by the candidate’s psychiatrist. The Clinical Director will also schedule a time for a visit and tour of our facility for the candidate and accompanying family member or clinician.

Fees
Fees for the Residential Living Center at Parents’ Foundation for Transitional Living, Inc. start at $4,903 per month. These fees exclude costs for medical expenses, personal care items, clothing and spending money.

Fee schedules are provided at the time of application. Fees may vary based on accommodations and level of service and some fees may be non-refundable.

Fees are generally paid by a combination of resident and family resources. A Fee Agreement signed by a responsible payor is required prior to admission.

Our experience has been that insurance coverage for our programs is rare. Financial aid is limited and will only be considered for those completing the Financial Aid application substantiating need.
